This form explains the risks of participating in a child development program and requires that the caregiver, the legal guardian/primary caregiver, agree to bear such risks. This form must be reviewed and signed by the caregiver, the participant’s legal guardian/primary caregiver, unless the participant is legally old enough to give their own consent, in which case he/she can provide his/her own consent. In such case the term “my child” shall be taken to refer to the participant themself.
